Few conversations in medicine are harder than deciding when to continue, withhold, or withdraw life-sustaining care. Dr. Joseph Varon brings clarity to the hardest parts: the difference between physiologic and medical futility, the ethical and legal framework that governs these decisions in the United States, and the principles that should anchor them. Just as important, he offers structured ways to actually have the conversation with patients and families, so it’s guided with both honesty and care.
Learning objectives
By the end of this session, participants will be able to:
- Define and differentiate the concepts of physiologic futility and medical futility, and apply these distinctions to clinical decision-making in critically ill patients.
- Distinguish between withholding and withdrawing life-sustaining therapies, including their ethical equivalence and practical implications in ICU practice.
- Analyze the legal framework in the United States governing end-of-life decisions, including informed consent, surrogate decision-making, and landmark cases such as Quinlan and Cruzan.
- Apply ethical principles (autonomy, beneficence, non-maleficence, and justice) to complex end-of-life scenarios involving critically ill patients.
- Develop structured communication strategies for initiating and conducting end-of-life discussions with patients and families, including goal setting and management of emotional responses.
